Official 5by5 Contest Rules for Awarding $100 Online Visa Gift Cards

1. NOTE: an opportunity to be entered into the random drawing to win a $100 Online Visa Gift Card is offered at the end of the survey.
2. A total of three (3) Online Visa Gift Cards (valid online wherever Visa® is accepted) will be awarded using a random drawing process.
3. Participant's contact information must be entered before the survey closes on 2/26/21 to be included in the random drawing. Contact information will be used only for the purpose of awarding prizes to the winners and will not be shared with anyone or used for any other purpose.
4. Participants are disqualified from the random drawing if they do not to provide contact information.
5. Only one entry per participant will be accepted.
6. Should you choose to not participate by offering your insights through the survey, you may still be entered into the random drawing to win one of the Online Visa Gift Cards.
7. No purchase is necessary to be entered into the random drawing.
8. Winners will be chosen through a process which utilizes a number randomizer, to include all participants who provide their contact information.
9. Winners will be chosen on 2/26/21. Participants will be contacted individually via email or phone, only if they are chosen in the randomized drawing.
10. The Online Visa Gift Cards will be delivered electronically (by email) to the randomly selected winners.
11. After winners are contacted and their prizes awarded, the winners are responsible for any unclaimed prizes, or the loss thereof.
12. The final chance of winning will be calculated and available upon request from 5by5 after 2/26/21.
13. A winner list can be obtained from 5by5 Research.
